I hereby release and waive the right to hold liable all sponsoring agencies, individuals, and Oak Hill Academy for any claim for injury or damages which may occur from participating in the Oak Hill Academy Basketball Camp. I certify that my child is in proper physical condition to participate in this camp. (Please note: a physical is not required for camp attendance.) For cancellations that occur more than two weeks before the start of your camp session, you will receive a partial refund. $100 of your camp tuition is non-refundable (and the 4% processing fee if you registered online). 2022 camp tuition is non-transferable to 2023 camp dates.
